Kapusta Hockey Academy uses Tally to operate certain online registration forms and collect information submitted by participants, parents, and guardians.

Information entered into a KHA registration form may therefore be transmitted to and processed or stored through Tally's systems on behalf of Kapusta Hockey Academy.

Kapusta Hockey Academy determines the information requested through its registration forms and how that registration information is used for KHA programs. Tally provides technology used to collect and process those submissions.

Registration information collected through Tally may be accessed by authorized Kapusta Hockey Academy personnel for purposes including:

Participants should not enter credit card numbers, bank account information, passwords, Social Security numbers, or other unnecessary sensitive financial or identification information into ordinary KHA registration fields.

If you are under 13, please do not independently submit personal information through our website or registration forms. A parent or legal guardian should complete the registration process on your behalf.

Stripe processes payment and transaction information when payments are made through KHA's Stripe-powered payment portal.
